Acute Postcataract Endophthalmitis Caused by Coagulase-Negative Staphylococcus Lugdunensis

白內障術後因路鄧葡萄球菌成染之急性眼內炎―病例報告

摘要


目的:報告白內障術後因少見之路鄧葡萄球菌感染所引發之急性眼內炎。 方法:病例報告-一名69歲女性糖尿病患者於右眼白內障術後第六天因視力糢糊眼睛疼痛紅前來求診。 結果:急性眼內炎一經確診後,即安排玻璃體切除術與玻璃體內注射抗生素。玻璃體與房水之檢體培養結果均證實為罕見之路鄧葡萄球菌感染。病患經治療並追蹤六個月後最佳矯正視力為20/50,且無其它併發症。 結論:路鄧葡萄球菌並非常見之凝固酶陰性葡萄球菌,卻仍可能導致嚴重急性眼內炎,囚此早期積極介入治療將可能恢復較佳視力結果。

Purpose: To report one case of acute postoperative endophthalmitis caused by Staphylococcus lugdunensis. Methods: A retrospective case report of a 69-year-old diabetic female who presented with blurry vision (hand motion), redness, and ocular pain in the right eye six days after cataract extraction. Results: Clinical diagnosis of endophthalmitis was made, and emergent pars plana vitrectomy with intravitreal antibiotics was performed. Cultures from the aqueous and vitreous fluids revealed the organism S. lugdunensis. The patient achieved a best corrected visual acuity of 20/50 without anatomical sequelae during the 6-month follow-up. Conclusion: S. lugdunensis endophthalmitis has clinical presentations different from endophthalmitis caused by other coagulase-negative Staphylococci. S. lugdunensis is a particularly aggressive coagulase-negative Staphylococcus species that may show an acutely fulminant course in patients with infectious endophthalmitis. Early intervention should achieve a favorable final visual outcome.
