Analysis of Ophthalmology Inpatient Consultation in a Regional Teaching Hospital

摘要

目前：分析區域教學醫院之住院病人眼科會診之形態、頻率與結果。方法：本回溯性研究期詞為2010年5月至2011年4月，對象為762位接受眼科會診之住院病人。我們紀錄就分析病入之性別、年齡、會診日期、住院科別、住院診斷、會診璁由、眼科診斷與特定之眼科治療。結果：病入平均年齡為55.4歲。要求會診科別主要為內科系（63.91%）、外科系（23.49%）、與兒科部（9.19%）。眼科會診璁由共74種，最常見的五種為紅眼（16.80%）、視力模糊（16.54%）、糖尿病視網膜症篩檢（16.14%）、早產兒視網膜症篩檢（5.38%）、與眼分泌物（4.86%）。眼科診斷共79種，最常見的五種為結膜炎（10.63%）、排除糖尿病視網膜症（8.01%）、非增殖性糖尿病視網膜症（8.01%）、無特殊發現（7.22%）、與增殖性糖尿病視網膜症（6.96%）。有16位病入在住院期間經眼科會診後，接受了特定的眼科治療。結論：醫院之眼科醫師對其他科別會診病入之眼科疾病評估與處理確有貢獻。住院會診病例亦有助於眼科住院醫師之訊練。本研究之結果或可作為未來教學計畫之參考。

Purpose: To analyze the type, frequency, and result of inpatient consultations with ophthalmologists encountered in a regional teaching hospital.Method: This retrospective case series reviewed the electronic medical records of 762 inpatients requesting an ophthalmology consultation from May 1, 2010 to April 30, 2011. We recorded patients' gender, age, date of consultation, admitting service, primary admitting diagnosis, reason for consultation, ophthalmology diagnosis, and specific ocular management.Result: The mean patient age was 55.4 years. Consultations were mostly requested by internal medicine services (63.91%), surgery services (23.49%), and pediatrics service (9.19%). Seventy-four different reasons for ophthalmology consultation were documented. The five most common reasons were red eyes (16.80%), blurred vision (16.54%), for diabetic retinopathy screening (16.14%), for retinopathy of prematurity screening (5.38%), and eye discharge (4.86%). Seventy-nine ophthalmology diagnoses were made. The five most common diagnoses were conjunctivitis (10.63%), diabetic retinopathy ruled out (8.01%), nonproliferative diabetic retinopathy (8.01%), no specific finding (7.22%), and proliferative diabetic retinopathy (6.96%). Sixteen patients received specific ocular treatments following ophthalmology consultation during hospitalization.Conclusion: Ophthalmologists in hospitals help assess and manage ocular conditions in inpatients from other departments. Inpatient consultation is also an abundant source of teaching material for ophthalmologists-in-training. The information from this study may help in designing residency teaching programs.
