Applying Laboratory Automated Reporting System on Infectious Disease Surveillance

摘要


實驗室自動通報系統(Laboratory Automated Reporting System, LARS)係結合醫院資訊系統,即時蒐集醫院端病原體檢驗資料,輔助流行疫情的監視。本研究利用實驗室通報的陽性檢體資料,與現行疾病監視系統進行趨勢分析,評估其監測病原體流行趨勢之可行性及於傳染病監測上之應用。在疾病管制署的常規監測系統中,LARS所監測的流感病毒以及輪狀病毒陽性檢體趨勢,與即時疫情監測及預警系統(RODS)及健保資料庫中類流感、腹瀉的就診病歷趨勢呈正相關(r=0.69~0.86)。LARS可輔助現行監測系統,提供可能引起目前流行疫情病原體的相關資訊。LARS同時收集病原體抗藥性的檢測結果,可做為未來國內抗藥性菌株現況之參考,惟此部分資料於防疫上的運用還需進一步的探討。

The Laboratory Automated Reporting System (LARS) was integrated with the hospital information system. This integration, does not merely allow the collection of data concerning pathogen detection in hospitals, but also enables real-time monitoring of epidemics. To assess the application of LARS on infectious disease monitoring and pathogen activity, we conducted a trend analysis by using the positive specimen data from the hospital laboratories and the information from the existing surveillance systems implemented by Taiwan CDC. Based on the routine surveillance systems implemented by Taiwan CDC, a positive correlation between the rotavirus and influenza virus activities and the trend of the visits for influenza and diarrhea by Real-time Outbreak and Disease Surveillance (RODS) and National Health Insurance medical records (r=0.69~0.86) was found by LARS. In this study, we summed up the advantages of utilizing LARS, including the collection of data on the drug resistance of pathogen, which could serve as a future reference for the trend of drug-resistant pathogens in Taiwan. Further studies will be needed to determine the application of certain data collected by LARS in disease prevention and control efforts.
