探討醫護人員之病人安全文化:以中部某地區醫院為例

STUDYING PATIENT SAFETY FROM VIEWPOINTS OF PHYSICIANS AND NURSES: A CASE OF A DISTRICT HOSPITAL IN CENTRAL TAIWAN IN 2017

摘要


正面的安全文化可以減少醫療疏失並且提供更安全及優質的照護環境,但依靠正面的組織文化是不足夠的,對於新成立的醫療機構而言,要快速提高競爭力,還需要執行標竿學習。本研究主要以2017年中部某新成立的地區醫院內部的醫護人員為研究對象,然而為了進行標竿學習,提供個案醫院能夠進步及改善的指標,因此加入了中部某兩家醫療機構(醫學中心及區域教學醫院)之醫護人員進行標竿測試。本研究採用財團法人醫院評鑑暨醫療品質策進會制訂的中文版病人安全態度量表量測病人安全文化,採用醫療機構每年固定進行的內部問卷調查,利用描述性分析、無母數Mann-Whitney U檢定及單因子變異數分析,瞭解個案醫院之醫護人員對於病人安全文化的各構面間的認知是否存在差異。利用單因子變異數分析將個案醫院及中部的兩家醫療機構進行分析。結果發現個案醫院目前所占的優勢是在安全風氣、工作滿意及工作生活平衡,而在事後檢定上發現個案醫院在工作滿意及工作生活平衡比其他醫院也有較好的滿意程度。透過調查及比較中部某新成立的地區醫院內部的醫護人員對於病人安全文化認知,瞭解醫護人員在不同構面裡的認知差異,以提供院方在病人安全上能即時做出改善及調整,避免醫護人員在工作過程中出現醫療疏失以維護病人安全。

Positive safety culture can reduce medical errors and provide a safer and better-quality care environment. However, relying on a positive organizational culture is not enough. For a newly established medical institution, benchmarking is essential in order to improve the competitiveness quickly. The research is mainly based on the physicians and nurses in a newly established district hospital in central Taiwan in 2017. However, in order to carry out the benchmarking and provide indicators for the progress and improvement of the case hospital, physicians and nurses from another two medical institutions (a medical center and a regional teaching hospital) in central Taiwan are included for benchmarking. This study uses the internal survey results from the Chinese version of the Safety Attitudes Questionnaire (SAQ) developed by the Joint Commission of Taiwan to measure patient safety culture in this case hospital. Descriptive statistics, nonparametric statistics, and analysis of variance are employed to evaluate whether there is a difference among the dimensions of the patient safety culture for physicians and nurses. A comparative study is performed with two additional medical institutions in central Taiwan through analysis of variance. The results show that the current advantages of the case hospital are in safety climate, job satisfaction, and work-life balance. In the post hoc analysis, it is found that the case hospital is more satisfied than the other hospitals in terms of job satisfaction and work-life balance. Through the investigation and analysis of the safety culture of the newly established district hospital in central Taiwan, hospital management can make immediate improvements and adjustments in patient safety to avoid medical negligence of physicians and nurses during the work to maintain patient safety.
