某地區護理人員照顧癌症患者臨床工作壓力之探討

摘要

本研究目的是在探討照顧癌症病患的護理人員臨床工作壓力源及相關影響因素。研究對象以立意取樣法，選取護理同仁177名。以結構式問卷進行調查，回收率達98%。採百分比、平均值、一方變異數分析、卡方檢定及迴歸分析，進行資料分析與探究。研究結果：一、護理人員對臨床工作壓力類別感受程度以病人照顧方面的壓力源最高，二、護理人員對各壓力源壓力感受較高的前五項排序爲：（一）暴露在有感染危險的環境中。（二）想要好好的處理病人問題，但是心有餘而力不足。（三）護理沒有醫藥常識的病人。（四）醫師對病人所做的檢查或治療事先不做說明，或解釋不夠清楚。（五）工作單位人力短缺、病人的病情再發反覆住院。三、專業知識、人際關係、工作負擔及行政組織等類別的壓力源均只受教育程度不同而有統計上顯著差異。四、病人照顧類別的壓力源與護理年資呈負相關。

The purpose of this study was to investigate stressors of the clinical nurses in taking care of cancer patients.
The subjects included 177 nurses who have been working at clinics or hopitals located in a metropolitan area for more than three months. A structured questionnaire was used for data collection and chi-square, one-way ANOVA, and linear regression were applied for data analysis.
The data showed that a) patient care accounted for the most stress on the clinical oncology nurse, and interpersonal relations was the least, b) the most stressful events were exposure to the risky environment, lack of knowledge in caring for the patient, knowledge deficit of the patient, physician’s negligence of explanation of the treatment procedures, and manpower shortage in perspective. c) Due to different educational backgrounds, there were significantly different feeling of stressors based on professional knowledge, working load and relationships among members. d). The more clinical experiences, the less stress from patient care.
This study implies that administrative support can be very helpful in releasing stress for the oncology nurse.
