居家護理病患常見護理問題及其護理過程之探討

摘要

本研究目的在探討南部某省立醫院居家護理病人所常見的護理問題，在提供居家護理服務三個月後，病患護理問題之進展情形。以回溯方式分析自民國78年6月1日成立至79年12月31日期間已連續居家護理三個月以上之病患及其主要照顧者各22名為研究對象。每位病患初次家訪時即盡量确立病患所有的護理問題，并指導主要照顧者設法解決其各項護理問題。內容分析每次家訪後之實際護理記錄，發現三個月期間改善最多病患的護理問題是「知識缺失」及「家庭運作過程改變」兩項；病患之護理問題嚴重程度有顯著改善的有「排便型態改變」「排尿型態改變」「飲食狀況改變」及「皮膚完整性受損」四項，分別以個案護理過程實例說明之。此研究成果可作為居家護理在職教育及居家護理人員照顧病患之參考。

The purpose of this study was to describe the common nursing problems in a hospital-based home nursing program, which was launched June 11989 in southern Taiwan. 22 families went through more than 3 months of the program. All nursing records were reviewed retrospectively. Content analysed and coded using ”Severity of Nursing Problem Scale” developed by researchers. Descriptive statistics were used SPSS/PC+. ”Knowledge deficit” and ”ineffective family coping” were the two most dramatically improved items after 3 months' home nursing.
The severity of ”altered bowel elimination”, ”altered pattern of urinary elimination”, ”altered nutrition”, and ”impaired skin integrity” had decreased significantly after three months home nursing.
Each nursing problem was discussed using case study as example to describe the whole nursing process. The implication of the study was to share the experience of home nursing in special nursing problems and as reference for staff development.
