摘要

本研究係以參與參察及深度訪談法，深入探討正常分娩的初產婦於待產及生產時的主觀經驗。在正式收集資料前，研究者由初產婦第一產程活動期開始，以其主護護理師的角色，一邊提供必要的護理服務，一邊深入觀察其行為。幷於產後8至24小時，和11位初產婦進行期小時左右一對一的深度訪談。訪談時產婦所表達的語言內容，即為本研究用以分析的資料。將253面的行為過程記錄予以內容分析，進一步分析與歸納的結果顯示，初產婦不同的主觀經驗有七類，包括不確定感、事事不相吻合感、面臨放棄或繼續支撐下去的抉擇、失去控制感、努力嚐試把持自我、努力重新裝備自我、達到完成及如釋重負感受等。本研究其他主題類似的研究相比較時發現：由於本研究所采用的研究法，不但能深入初產婦分娩當時的情境和主觀世界，幷能使研究者更易與產婦建立良好的關係，故所獲得的資料也較其他研究更為深入。

The main purpose of this study was to explore the subjective experience and nursing needs of eleven primipara mothers who were in the process of normal spontaneous labor and delivery. During the course of labor and delivery, the researcher served as a care-giver and nurse-participant observer, then conducted a one hour indepth interview 8-24 hours after delivery. The researcher wrote down a narrative process recording from verbal and non-verbal behaviors expressed by these primipara mothers during interviews. By means of content analysis, the verbal behaviors of subjects were systematically analyzed and categorized.The results found that the subjective experience of the primipara, according to the progressing of the labor and delivery course can be categorized as following: sense of uncertainty, sense of mis-matchedness, coming to a crossroad of giving up or continuing, sense of loss of control, trying to hold on to self, engaging in re-equipping self, sense of accomplishment and sense of relief.In addition, compared with other similar studies, this study provides more profound information. As result of the methodology adopted in this study, the researcher could confront the subjects' situation and understanding of real world. Meanwhile, the researcher was able to establish a good relationship with the subjects.
