Taiwanese Patients' Expectations for Recovery from Cardiac Surgery during Preoperation Period

心臟手術病人於術前住院期間對恢復的期望

摘要


接受心臟手術的台灣病人對恢復的期望,至今仍所知有限。本研究爲一大型研究計劃的一部份。透過半結構式訪談與作者所創之綜合“基礎理論”(grounded theory)、“內容分析”、“資料連接”與“經常比較法”之質性分析法,本研究首次探討病人在心臟手術前的住院階段中,對術後恢復的期望。 本研究採“理論取樣法”(theoretical sampling)自台灣北部三家醫院中接受心臟手術之患者取樣30名(15名男性,15名女性)。研究發現接受心臟手術的病人對恢復的期望概念包括:(1)恢復的定義,(2)恢復的程度,(3)恢複的內容,(4)恢復的時間表,及(5)影響恢復的因素。病人預備或已經用來促進術後恢復的策略包括:(1)運用個人的力量,例如做體操、每日散步、經常性活動肢體、或限制活動以保持體力、改變暴飲暴食的習慣,與安排辦公室工作,(2)運用他人的幫助,例如:親友的陪伴,與事先通知加護病房護理人員保護,及(3)運用超自然力量的幫助。

Few data are available concerning Taiwanese patients’ expectations for recovery from cardiac surgery. As part of a larger study, Taiwanese patients’ expectations for cardiac surgery recovery during admission and prior to surgery were explored through semi-structured interviews and analyzed qualitatively by using a combination of grounded theory, content analysis, linking data, and constant comparison. A theoretical sample consisting of thirty (15 men, 15 women) adult patients having cardiac surgery was obtained from three hospitals in northern Taiwan, Republic of China. The concept of expectations for recovery was found to encompass the definition, level, components, time frame, and context of cardiac surgery recovery. Strategies developed to prepare for or facilitate recovery included: (a)the use of person-focused effort, such as doing gymnastics, taking a daily walks, frequent body movements or limiting physical activities in order to preserve physical energy, changing eating habits (eating and drinking gluttonously), and arranging office works, (b)getting help from others, such as companionship from family members and optimistic friends, and calling the ICU nurse for help, and (c)using metaphysical power.
