血管性巴金森癥候群在氟—18—FDG腦部掃描的表現:初步報告

Vascular Parkinsonism in FDG-PET Scan: Preliminary Study

摘要


背景:本初步研究之目的是要以氟-18-FDG正子造影來探討血管性巴金森症候群(VP)病患之基底核及丘腦是否有葡萄糖代謝異常的情形。 方法:本研究共收集6位血管性巴金森症候群(年齡範圍:48-85歲,平均:70±14.3歲)。4位巴金森氏症(PD)(年齡範圍:59-71歲,平均:68.5±6.6歲)的病患。4位年齡相符的健康自願者(年齡範圍:53-64歲,平均:57.5±5.1歲)當作對照組。所有受檢者接受氟-18-FDG正子腦部檢查。從每一位受檢者得到之腦部橫切面(transaxial slices)當中挑選基底核最明顯的兩個切面。手動選取基底核及丘腦之ROIs。ROIs的形狀為圓形,直徑為5mm。再將相同區域ROIs中SUV(standard uptake value)平均後,各組進行比較。 結果:VP一組病人不管是在基底核及丘腦部位(SUV介於4.09及6.51之間)都比正常對照組(SUV介於5.93及8.59之間)或PD這組病人(SUV介於5.61及9.94之間)來的低(P<0.02)。正常對照組和PD這組相比,經由Mann-Whitney test分析後,不管是在基底核及丘腦部位,在統計學上兩者並無顯著差異(P>0.05)。 結論:氟-18-FDG正子造影可以幫忙作VP及PD之鑑別診斷。

Background: The aim of this preliminary study was to determine whether there was metabolic change in the basal ganglia or thalamus in the vascular parkinsonism (VP) patients by 18F-FDG-PET scan. Methods: 6 VP patients (age range: 48-85 years; mean age: 70±14.3 years), 4 Parkinson’s disease (PD) patients(age range: 59-71 years; mean age: 68.5±6.6 years), and 4 normal controls (age range: 53-64 years; mean age:57.5±5.1 years) were enrolled in this study. 18F-FDG-PET brain scan was performed in each patient. Five-millimeter circular regions of interest (ROIs) were used to sample radiotracer specific uptake value (SUV) in the selected basal ganglion and thalamus regions. The mean data of SUV in each selected area were compared between groups. Results: Mann-Whitney test was applied in this study. The SUVs in VP patients (4.09-6.51) were statistically significant lower in bilateral basal ganglia and thalamus regions than in normal controls (5.93-8.59) and PD patients (5.61-9.94) (P < 0.02). There was no significant difference in uptake of 18F-FDG in the basal ganglia and thalamus regionsas compared with PD from normal controls (P > 0.05). Conclusion: Evaluation of glucose metabolic rate at bilateral basal ganglia and thalamus using 18F-FDG PET is valuable in differentiating VP from PD.
