Mimicking Extensive Bone Metastases in FDG PET Scan in an Oral Cancer Patient with Polyostotic Fibrous Dysplasia

摘要


在頭頸部癌症中,骨轉移是第二常見的遠端轉移。氟-18-去氧葡萄糖正子斷層攝影被證實可應用在偵測頭頸部癌症的原發部位,淋巴結及遠端轉移或是繼發性腫瘤的存在。於此,我們報告一位頭頸部癌症在骨骼系統表現有多處不正常去氧葡萄糖聚積的病例。經由比較多次骨骼掃描的結果,臨床追蹤,及正子掃描影像的型態表現,這些不正常的去氧葡萄糖聚積被認為是多發性骨纖維性再生不良而不是多處骨轉移。這是第一篇有關葡萄糖正子斷層攝影在頭頸部癌症中,同時並存有多發性骨纖維性再生不良的病例報告。

Bone metastases are the second most frequent site of distant metastasis in head and neck cancer. It has been shown that 18F-2-fluoro-2-deoxy-D-glucose (FDG) positron emission tomography (PET) is able to identify primary lesion, lymph node involvement, and distant metastasis or other secondary tumor in head and neck cancer patients. Herein, we present a case of head and neck cancer with multiple abnormal FDG accumulation in the skeletal system. The abnormal FDG uptake was considered as polyostotic fibrous dysplasia, instead of multiple bone metastases depicting from a series of previous bone scans, clinical follow up and presentation of morphologic pictures in FDG PET studies. To the best of our knowledge, this is the first case report of the coexistence of polyostotic fibrous dysplasia and head and neck cancer on FDG PET scan.
