核子醫學造影檢查電腦排程曁報告系統之設計與臨床使用

System Design and Clinical Applications for a Nuclear Medicine Imaging Study Management and Reporting System

摘要


核子醫學造影檢查的特色就是每一種檢查都有各自的流程及時間表,但也常因爲這些差異容易造成流程中各科醫務人員間溝通上的誤解,衍生後續如受檢者等候的時間浪費、額外的藥物劑量等等問題。本研究擬以電腦資訊系統的開發,經向合理的流程管控及透明的相關資訊來增加核醫檢查的妥善率,並整合各階段的資訊以提供醫師報告對檢查流程有全面性掌握。本「核醫排程系統」(Nuclear Medicine Client Manager; NMCM)在醫療資訊系統內定位爲核醫科的排程報到及報告系統,對上連結醫院的「醫療資訊系統」(Hospital Information System; HIS)及「影像擷取及傳輸系統」(Picture Archiving and Communication System; PACS),對下則在核醫科內架設一部MySQL伺服器作爲本地資料庫(local database)。系統使用的程式語言爲Microsoft(上標 ®) Visual Basic 6.0中文版,本地資料庫爲MySQL 5.0.27(封裝於AppServ 2.5.6)。NMCM包含排程、醫師確認排程、藥局、護理、造影及報告共六個子程式及一個維護程式。資料庫間以Structured Query Language (SQL)語言溝通。硬體方面,本地資料庫以Intel(上標 ®) Pentium(上標 ®) D, 3.0 GHz CPU, 1GB RAM之個人電腦運行,並使用兩顆250 GB SATA硬碟組合成RAID-1磁碟陣列以維護資料安全。本套軟體系統自2006年2月起應用於本科,並於2007年6月起應用在另一分院。服務超過14,000名的病患檢查,包含:心肌灌注掃描、全身骨骼掃描、正子造影箏。本系統更提供研究搜尋、計算、與套裝報告格式等輔助功能。本套軟體系統已穩定上線服務2年以上,整合臨床與檢查資料以提供報告醫師檢查流程的全面性資料。資料庫可以提供研究病例搜尋功能,符合核子醫學部門醫學資訊管制流程的及時性與正確性。

One of the distinctive features in nuclear medical study is that each type of the examination has its unique procedure and scheduling, as a result, communication error often causes unnecessary patient waiting and extra radiopharmaceutical consumption. This study aims at developing a software-based system in resolving these issues and also providing an efficient report-filing mechanism. The in-house developed ”Nuclear Medicine Client Manager” (NMCM) is a client schedule registration, and report system which uplinks to Hospital Information System (HIS) and Picture Archiving and Communication System (PACS) and downlinks to a local database which uses MySQL for saving all data. The User Graphic Interface was developed by Microsoft Visual Basic 6.0 Chinese version on Microsoft Windows XP Professional operation system. NMCM comprises six subprograms, including Schedule, Doctor Schedule, Pharmacy, Nurse, Scan and Reporter. The entire database communicates by SQL, and the local database runs on a PC equipped with Pentium D 3.0GHz CPU and 1 GBytes RAM. The system utilizes two hard disk drivers which are combined as a RAID-1 hard disk drivers array for data protection. This system commenced its operation at February, 2006 in our department, and another hospital also started using it in June, 2007. It has since served more than 14,000 patients' imaging examinations, such as myocardial perfusion scan, whole body bone scan and PET scan, et al. In addition, this system also supports certain enhanced function such as data searching, dose calculating and report generating for user’s convenience. This system has demonstrated high stability and minimum maintenance after two years of field-testing. It has improved the satisfactory rate of nuclear medicine examination, and integrated clinical and examination data for nuclear physician to better control the examination flow. The database generated also provides various types of information for specific searching condition to meet the calculation and statistical requirements.
