摘要

在緊急醫療救護法尚未公怖實施以前，我國的緊急救護還一直停留在「載了就走」的處理模式之中，而今；緊急醫療救護法己完成立法，專責機構消防署也成立了。消防署委託各大教學醫院，對消防隊員實施初級、中級救護技術員訓練，務使緊急醫療救護品質得到最快的提升。關於訓練課程時數的安排上；學員則傾向於增加醫院實習時數，而且也認為醫院實習才是對他們實際救護工作最有幫助的訓練課程；另外在訓練前後，對緊急救護的九大技能（基本救命術、給氧、包紮、骨折固定、頸圈固定、協助生產、身體評估、哈姆立克急救法及半自動體外去顫器之使用），做為學員的自信心及能力評分，用paired-t test分析結果，確實有明顯的增進，這實在是可以肯定我們的訓練成效及學員的努力。但仍然要根據學員所提出的建議，再加以改進，以求更完美的安排。

In Taiwan patients arriving by ambulance in Taiwan lack prehospital advanced life support (ALS) leave care. Determining the need for increasing public access to such care is urgent. It is important to collect local data regarding present emergency medical technician (EMT) raining programs, in order to direct our furture planning and to establish an optimal EMT training program.
The EMT-intermediate training course at VGH Taipei Taiwan includes: a 3 weeks lecture courses, a 1 week hospital clinical practicum and an 8 days ambulance clinical practicum.
The EMT-intermediates were tested for their ability and confidence in basic life support, oxygenation, wound package, fracture fixation, neck fixation, hysical examination, emergency department delivery, Heimlich maneuver and defibrilation before and after this EMT-intermediate aining program.
The EMT-intermediate candidate median scores before training were 10.63±3.84 (mean ± SD), and 16.67±1.68 after training. The EMT-intermediate confidence scores before training were 13.22±3.75, and 15.78±2.31 after training. Both reached stastistical significance between by paired-t test, and indicated that the training was helpful.
The data in this article were obtained from EMT's trained at Taipei VGH. The trainees expressed a preference for the hospital clinical practicum, considering it to be the most useful.
