對於神經重症病患之監測系統發展迅速,而經顱都卜勒超音波扮演著舉足輕重的角色,其「即時、安全、經濟、可重覆執行與連續性監測」等特性,可提供與電腦斷層互補的資訊,且有助於臨床決策與用藥的調整。本文回顧經顱都卜勒超音波的發展、原理、操作,探討使用在頭部外傷、腦中風等神經重症照護時的臨床意義,並提出三個案例介紹它的臨床應用。
Transcranial Doppler (TCD) and transcranial color-coded duplex sonography (TCCS) play a significant role among monitor systems in current neurointensive care. They provide real-time, safe, economical, reproducible and continuous monitoring of cerebrovascular characteristics in patients hospitalized in neurological and neurosurgical intensive care units. This article reviews the development, principle and use of TCD and TCCS, elaborates the clinical significance of the data obtained from patients with head injury and cerebrovascular events, and demonstrates their clinical use with three cases.
