職業醫學與醫療服務投資之相關決策探討

摘要

醫院在面臨總額預算制度的實施，經濟不景氣、政府財政窘困，公立醫院補助不足及醫療糾紛事件頻傳，醫療品質受到威脅的多重壓力下，使得醫院的經營管理階層在做投資決策時，必須要同時面對提高醫療品質、與降低醫療成本做妥善分析評量與決策。本文以醫院在職業醫學各項健檢中，為使醫院拓展更多元的服務，與建立醫院的優質形象，做決定服務拠資前，必需仔細衡量各種財務因素（成本效益）與非財務因素（教學研究、服務品質、服務流程之便利性）。並應在最佳成本效益與服務品質前提下，找尋最優良的醫院為標竿，訂定一可長可久的決策方案，讓醫院的服務能在競爭的市場中，維持其應有的品質與民眾的信賴，使日益受重視的職業醫學能為民眾的健康，做最大的貢獻。

Hospitals and healthcare systems are facing increased financial difficulties because of the global budgeting system ,a slump in the market, insufficient subsidy, and increased malpractices. The continuing uncertainty has led health care mangers to evaluate the investment decision-making with great care based upon care of quality and medical cost. This study took the check-up services in the occupational medicine as an example. In terms of cost-effectiveness and care of quality, while making a critical investment decision, financial factors (costs) as well as non-financial factors (medical research ,service quality, and service protocol )should be concerned and analyzed prudently, making hospitals in balance between controlling cost and maintaining quality patient care.
