護理人員於負壓病房照顧肺結核病患之風險評估初探

摘要

護理人員為職業傷害及職業疾病之高危險群，由於為第一線照顧及密切接觸病患者，對於傳染性疾病應該更注意防範。本文探討在負壓隔離病房，照顧肺結核患者之護理人員，其所接觸肺結核病患情況及其健康情況，經統計一年內結核病患198位中，高達3187日總住院日，其中10%為使用呼吸機之重症患者。24位護理人員一年當中，有六人次出現發燒、腸胃炎、上呼吸道感染、咽喉炎等感染症狀，但皆無肺結核之職業性感染發生。所以在提供設計完善的負壓病房、防護衣、N95口罩、洗手設備等感控防護，於正常的健康護理人員，在合理的工作時數內，可以得到完整的防護，於工作的心理壓力下可得到紓解及慰藉，使工作人員於心理及生理達到最佳的保障，避免護理人員的流失及感染結核病之危險。

Nurses are the high risk group in occupational hazard, especially in the ward caring patients with infectious diseases, such as pulmonary tuberculosis. This study was designed to evaluate the infectious risk of nurses caring patients with pulmonary tuberculosis in the negative-pressure ward. In this 1-year retrospective study, there were 198 pulmonary tuberculosis patients with 3187 hospital days in total. Among these patients, 10% of them were ventilator dependent. During the one-year period, there was no nosocomial infection with pulmonary tuberculosis among the 24 healthy nurses workers, even 6 events for nurses got fever, gastroenteritis, acute respiratory infection and acute pharyngitis. It is important for preventing respiratory infection with providing standard precaution including intact negative-pressure ward, and N-95 mask. Better equipments and training in control infection are the basic safety, for nurses working in the high-risk infectious ward.
