潛艦及潛水人員試壓體檢之醫學問題探討
The Investigation of Medical Problem Diving Pressure Tolerance Test in Navy Submariners and Divers
摘要
本研究之目的，在研究和探討海軍新進潛艇和潛水人員於體檢甄試時，因接受高壓艙氣體壓力試驗（50磅/吋^2即4.4大氣壓）所產生的醫學問題。672位海軍新進潛水人員接受壓力試驗，統計結果如下：(1)年齡：18-24歲，平均22.9±3.2(SD)歲。(2)教育程度：國小及以下0%，國中17.9%，高中(職)76.4%，大專及以上5.7%。(3)個人疾病史：無特殊疾病史。(4)壓力試驗結果：未通過百分比：12.3%；其中二五六戰隊佔15.7%，兩棲偵搜大隊佔10.3%，一九二戰隊佔1.7%，爆破大隊佔8.5%，特勤二中隊佔10.7%。(5)未通過壓力試驗原因：擠壓傷害佔70.9%，心理因素及其他佔23.3%，鼻出血佔3.5%，變壓性眩暈佔2.3%。結論：在軍方的嚴格選兵醫學下，我國之海軍新進潛水人員的特點是：年紀輕、教育程度高、無特殊疾病史；他們在接受潛水訓練前，會先至具備高壓艙之軍醫院接受身體檢查，及壓力試驗。若有上呼吸道疾病者，則先排除以免造成耳朵不適。未能通過壓力試驗者計12.3%；其中70.9%為擠壓傷害，心理因素及其他為23.3%，3.5%為鼻出血，2.3%為變壓性眩暈。

This study was aimed to investigate medical problems of hyperbaric test (50pounds/feet^2, 4.4ATM) while newly navy divers’ physical examination. 672 Navy newly divers were included in this survey study and consequent statistical results were as follows: (1) Age: 18-24 yrs, mean 22.9±3.2(SD) yr; (2) Education: primary school and below 0%, junior middle school 17.9%, high school and occupational school 76.4%, college and university 5.7% (3) Diver's personal history: nil (4) The results of pressure test: Not pass of all:12.3%；Navy 256 squadron: 15.7%, Amphibious reconnaissance patrol(R.O.C. marine):10.3%, Navy 192 squadron: 1.7%, Underwater demolition unit (R.O.C. Navy): 8.5%, Special service company (Chinese marine Corps):10.7% (5) The reason of not passed pressure test: squeeze:70.9%, psycologic and others:23.3%, epistaxis:3.5%, alternobaric vertigo:2.3%。 In conclusion, our newly navy divers are young, highly educated, and no special personal history. Before they taking diving training, they will receive physical examination and pressure test in military hospital with hyperbaric chamber. Not passed pressure test: 12.3%; 70.9% of them are squeeze, 23.3% of them are psycologic and others, 3.5% of them are epistaxis, 2.3% of them are alternobaric vertigo.
