護理床邊交班之時間及內容分析

A Study on Efficiency and Contents of Bedside Handover

摘要


交班的目的是在維持病人照護的連續性,不僅培養護理人員的團隊精神,同時也保障病人的安全,然而卻是一項必須投注很多時間的工作。本研究是以參與式觀察法記錄床邊交班的情形,主要是要瞭解交班所需的時間、內容及其影響因素。本研究自95年8月29日至8月31日止在一所醫學中心的三個內外科病房進行資料收集,總計有339筆交班資料。結果發現:(1)在交班時間方面:大夜班與白班交班所花費時間最長,但平均花費在每位病人的交班時間相同。經由ANOVA檢定發現年資在一年以下的護理人員,在交班上所花費的時間顯著多於5年以上者。(2)在交班內容方面:每筆交班均具有提供訊息的功能,但是在社會及教育功能上則較少。1年以下的護理人員在交班時需要被教育或是對交班內容作澄清的機會多於年資5年以上的護理人員。本研究建議護理人員應學習如何交班,以改善交班的技巧,並且輪流執行交班,以減少交班時所出現的干擾。

Shift handover plays a pivotal role in the continuity of patient care. It not only builds a sense of a team, but also a safety environment for patients. Yet, it is a time-consuming work. The purpose of this study was to explore the efficiency during bedside handover in three medical-surgical wards of a medical center in Taiwan. Participant observation was used to collect data between August 29 and August 31, 2006. A total of 339 handover data were included. The results showed that: (1) Night and daytime shift handover took the longest period in three shift handovers. The average time nurses spent in three shift handovers for each patient had no significantly different. However, nurses who worked less than 1 year spent much more time than those who worked more than 5 years. (2) All handovers involved informational function, but less in social and educational function. But nurses who worked less than 1 year spent more time in clarifying handover contents or receiving education from senior nurses. Besides, handover were sometimes interfered with answering phone calls or sending patients to checking. Therefore, this study implicates that junior nurses need to receive training to improve their handover skills and all nurses should take turns in shift handover to decrease the interferences during handover.
