血清C反應蛋白與各種肥胖指標的相關性:以台灣某醫院體重過重員工為例

The Relation between High Sensitivity C-Reactive Protein and Different Obesity Indices in Overweight Middle-Aged Hospital Employees

背景與目的:高敏感C-反應蛋白(hs-CRP)為身體低度發炎指標,與動脈粥狀硬化的過程有關。文獻顯示肥胖者有較高的hs-CRP。本研究的目的在探討體重過重的族群其hs-CRP與各種肥胖指標的相關性。 材料與方法:收集某綜合醫院員工體重過重者,身體質量指數(BMI)大於等於24kg/平方公尺331人參與本研究,進入健康促進計劃,以問卷填寫方式登錄個案基本資料和抽菸習慣,並測量身高、體重、腰臀圍、體脂肪百分比與血壓,血液檢查包含空腹血糖值、hs-CRP及血脂檢查等。 結果:在多變項線性迴歸統計方法之下,身體質量指數、腰圍、體脂肪百分比各別與hs-CRP做相關分析,控制各種相關危險因子(年齡、抽煙與否、空腹血糖、收縮壓、舒張壓、血脂)之後,發現無論性別,各種肥胖指標都分別與hs-CRP成顯著的相關性。但將身體質量指數、腰圍、體脂肪百分比一同放入多變項線性迴歸中,控制各種相關危險因子之後,發現在男性,身體質量指數與體脂肪百分比都與hs-CRP成顯著相關;但在女性,只有體脂肪百分比與hs-CRP成顯著相關。 結論:無論性別,在各種肥胖指數當中,以體脂肪百分比與hs-CRP有最強的相關性。

Background: High sensitivity C-reactive protein (hs-CRP) is a marker of low- grade inflammation and associated with the process of atherosclerosis. Recent studies have demonstrated that obese people have higher hs-CRP. Therefore, the goal of our study is to discuss the relation between hs-CRP and different obesity indices on the overweight but healthy group. Methods: The participants were recruited from the employees of a certain general hospital for health promotion activities, and they were composed of 331 anticipants who were overweight with body mass index (BMI) larger than 24kg/m^2. Medical history, life style, physical examination, body weight and height, waist and hip circumference, blood pressure and laboratory tests including fasting glucose, lipid profile and hs-CRP were assessed. Results: Multiple linear regression analyses were performed using interactions between BMI, waist circumference and body fat percentage respectively for each gender with hs-CRP as the outcome. We discovered that every obesity index has a significant association with hs-CRP regardless of gender after adjusting for age, smoking status, fasting plasma glucose, systolic and diastolic blood pressure, and lipid profile. However, after we put BMI, waist circumference and body fat percentage into multiple linear regression at the same time, we found that both BMI and body fat percentage were significantly associated with hs-CRP in men. But in women, only body fat percentage was significantly associated with hs-CRP. Conclusion: Among different obesity indices, body fat percentage was the most significantly associated with hs-CRP regardless of gender.
