異常氣壓環境作業人員接受耐壓及耐氧測試之健康評估

Health Evaluation of Dysbaric Environmental Worker Receiving the Pressure and Oxygen Tolerance Test

摘要

依照「勞工健康保護規則」，異常氣壓作業勞工必須接受在壓力每平方公分兩公斤以上的條件下，呼吸純氧三十分鐘之耐壓耐氧試驗。本研究收集民國97年1月至12月，至本院潛水醫學部之多人高壓艙接受耐壓及耐氧測試者共計837人，男性799人(95.5%)及女性38人(4.5%)，平均年齡24.3±4.9歲（範圍：18-52歲），教育程度主要為大學492人(58.8%)及高中職278人(33.2%)。試壓目的包括軍方新兵甄選700人(83.6%)，軍方年度體檢120人(14.3%)，民間年度體檢9人(1.1%)及民間考職業潛水執照8人(1.0%)。結果顯示參與試壓深度60呎有49人(5.9%)，其中10人(1.2%)未通過；112呎有706人(84.4%)，其中53人(6.3%)未通過；165呎有82人(9.8%)全部通過。試壓未通過大多發生於深度0-10呎有26人(3%)，其次為深度11-20呎及21-30呎分別各有13人(1.5%)。試壓未通過原因主要為耳膜擠壓傷有63人(7.5%)，其中TEED分類0級有21人(33%)，1級有34人(54%)，2級有8人(13%)。試壓前有病史者計42人皆未通過測試，其中以急性上呼吸道感染為最多有25人(59.5%)，過敏性鼻炎有9人(21.4%)，中耳炎有8人(19.1%)；而無病史者有795人，其中21(2.6%)人未通過測試。由此研究結果可作為勞工安全衛生主管機關或國防部軍醫局對於異常氣壓作業人員之甄選參考。

According to the health protection regulations of labor, dysbaric environmental worker must pass the pressure and oxygen tolerance test which is carried out under the pressure of two kgs per square cm and breathing 100% oxygen for 30 mins. This study collected 837 subjects who received the pressure and oxygen tolerance test in the multiplace hyperbaric chamber at our hospital from Jan. 2007 to Dec. 2007. There were 799 males (95.5%) and 38 females (4.5%) with a mean age of 24.3±4.9 years (range, 18-52 years). Their levels of education were mainly undergraduate (n=492, 58.8%) and high school (n=278, 33.2%). The purposes of the test included recruits selection (n=700, 83.6%), military physical exam. (n=120, 14.3%), civilian physical exam. (n=9, 1.1%), and diving license (n=8, 1.0%). Fourty two subjects who had diseases failed to pass the test. The diseases included acute URI (n=25, 59.5%), allergic rhinitis (n=9, 21.4%) and otitis media (n=8, 19.1%). In 795 subjects who had no diseases, 21 (2.6%) subjects failed to pass the test. These results may provide the useful information for the military selection of the recruits who work under the dysbaric environment.
