健康照護從業人員之職業性疥瘡：個案報告暨文獻回顧

勞工因作業環境中暴露於疥瘡，導致出現疥瘡相關的臨床證據，稱為職業性疥瘡（occupational scabies），疥瘡在職業醫學的領域中，自古就有廄癢（barn itch）及騎兵癢（cavalryman's itch）的文獻記載，然而職業性疥瘡在現代化國家之中，卻更好發於健康照護從業人員（health care workers），尤其在長期與病患直接皮膚接觸的長照機構（long-term care facility）中。疥瘡雖然不會致命，但影響勞工與家屬身心甚鉅。在長期照護產業蓬勃發展及從業人員日漸增多的台灣，職業醫學科醫師必須對職業性疥瘡多所了解，才能防堵疫情並給予勞工妥善的治療措施。本文將報告一例罹患職業性疥瘡的照顧服務員，並回顧健康照護從業人員之職業性疥瘡相關文獻，以期提供職業病科醫師早期診治及預防。

Occupational scabies is a human infectious disease caused by workplace exposure to Sarcoptes scabiei with sufficient scabies-associated clinical evidences. Scabies, an occupational disease that has affected humans from ancient times, were described as barn itch and cavalryman's itch in the literature. However, in the modern country, the occupational scabies occurred in health care workers, which were caused by direct skin-to-skin contact with residents in long-term care facility. Although scabies infestation is not life threatening, it certainly has a great influence on the workers and their family’s daily living. Occupational scabies had a dominant role in occupational infectious diseases among health care workers and other staffs in long-term care facilities. Under the circumstance of the increase of long-term care facilities and health care workers, occupational physicians needs to have more understanding on occupational scabies to protect workers from the infestation and to provide appropriate treatments for this disease. A case of occupational scabies in chronic health care institution is reported in this article. We also review relevant literature on occupational scabies among health care workers. Hopefully, it can provide ways to occupational physicians in early diagnosis and the treatment of occupational scabies.
