探討台灣醫療人員常用醫用口罩之防護效果與其使用滿意度

Investigation the protective effect of the common medical masks and the satisfication of the medical users in Taiwan

摘要

醫用口罩是臨床第一線照顧病人的醫療人員最常用來防止由呼吸感染之病原的工具，但台灣並未有研究進一步探討醫護人員配戴口罩後的防護效果與舒適感。故本研究比較不同類型醫用口罩對於醫療從業人員之個人安全防護的成效與舒適感。本研究為立意取樣，採橫斷式研究之類實驗與交叉研究設計，選取30名於台灣某醫學中心任職且需進出細菌性肺炎病人病室之醫療人員，隨機分派至4組，每組皆接受4種不同之口罩，進行口罩滿意度問卷調查及口罩細菌培養。研究結果顯示口罩使用滿意度，除傳聲功能受阻情形四種口罩並無顯著差異外（p=0.438），在整體滿意度、舒適度、悶熱度、防霧程度、呼吸阻力、視野受限方面，防霧、活性炭、一般醫用與外科手術四種口罩都有顯著差異（P＜0.05），防霧口罩顯著高於活性炭、外科與一般醫用口罩。醫療人員在接觸性皮膚炎程度上，四種口罩無顯著差異（p=0.128）。在數種稀釋倍數下，活性炭、一般醫用、外科與防霧口罩菌落數皆為零。防霧型口罩不論在舒適度、悶熱度、防霧程度、呼吸阻力、視野受限與整體滿意度方面皆高於活性炭、一般醫用與外科手術口罩。在本研究中特定的環境及行為下，四種口罩皆可有效阻隔細菌。

Medical masks are the most often applied tool to protect respiratory infection by clinical staff. However, there was no study investigated in the protective effeicient and the comfortability of medicl staff after wearing medical masks in Taiwan. Therefore, this study aimed to compare the protective effeicient and the comfortability of different kinds of medical masks. This study was cross-sectional, cross over and quasi-experimental design. 30 medical staff working at a medical center in north Taiwan and have to provide medical service for the patients with bacterial pneumonia were randomly allocated into 4 groups by purposive sampling. Each group received 4 different kinds of masks and wrote the mask satisfaction questionnaire after they wear a kind of mask, and the masks were sent to bacterial culture right away after the masks were removed. The study showed there were no difference in the safisfication of sound transmission function between groups (p=0.438). There were significant differences in the satisfication of overall, comfortability, sultry, anti- fog, respiratory resistance, and limited visual field among anti-fog mask, activated carbon mask, medical mask, and surgical mask(P＜0.05), anti-fog mask was better than the other masks. Anti-fog mask, activated carbon mask, and surgical mask had higher satisfication of adjustable strap than medical mask (P＜0.05). There were no significant differences in the severity of contact dermatitis among 4 masks(p=0.128). There were no colony generated in all of masks in different concentration of petri dishes. Anti-fog mask was better than the other masks in the satisfication of overall, comfortability, sultry, anti-fog, respiratory resistance, and limited visual field. All of the anti-fog mask, activated carbon mask, medical mask, and surgical mask could block bacterial effectively in the specific environment and behavior of this study.
