Brain Necrosis Related to Irradiation for Pituitary Adenomas

腦下垂體腺瘤經放射線治療引致之腦部壞死

從1987年11月至1993年7月,共有48位腦下垂體腺瘤病患在本科接受放射線治療,其中有二位分別在治療結束後16個月及5個月時發生了腦部壞死症狀,此二位病患於腫瘤部位接受了術後一仟萬伏特(10MV)光子射線的高能量體外放射線治療,總劑量分別為56格雷及58格雷。磁振造影檢查及臨床表現均符合放射綫引起的腦部壞死,然而均無病理證實。本文幷略加討論腦下垂體腺瘤經照射後引起腦部壞死之發生率及其病理生理發生過程。

Between November 1987 and July 1993, two out of the forty-eight pituitary adenoma patients treated with radiation therapy developed treatment-related brain necrosis. They had received post-operative external irradiation to the tumor area using 10MV X-ray with total dose as 56Gy and 58Gy, respectively. Unfortunately, brain necrosis symptoms occurred 16 and 5 months after the completion of treatment. Magnetic resonance imaging and clinical presentations correlated with radiation-related brain ne-crosis, however, tissue proof was not performed. The incidence and pathophysiology of the radiation-related brain necrosis was discussed and reviewed.
