運動訓練對改善胰島素阻抗及調節脂締素機制之探討

摘要


人體血液中的脂締素(adiponectin)與體脂肪含量、血脂異常及代謝症候群指標具有高度的相關性,尤其是中心型肥胖、胰島素阻抗(insulin resistance)、第二型糖尿病及心血管疾病患者常伴隨有低脂締素血症(hypoadiponectinemia)的現象。而有氧運動或肌力訓練,爲肥胖或第二型糖尿病患者,改善其胰島素阻抗及代謝不良指標的運動處方。數週的運動訓練計畫,對於脂締素濃度及脂締素受體(receptor)表現的提升,具有直接或間接的促進效果,可能藉由活化細胞內腺苷單磷酸活化蛋白激酶(5'-AMP-activated protein kinase, AMPK)之訊息傳遞路徑,促進肌肉細胞之粒腺體數量及氧化酵素功能,進而增加胰島素的敏感性及改善血脂狀態。因此,肥胖或胰島素阻抗族群,可藉由運動訓練活化脂締素訊息傳導路徑,提高胰島素敏感性,以降低罹患心血管及代謝疾病的風險。
