橫弓對於足部勁度與前足傷害之關係

The relationship between transverse arch and foot stiffness and forefoot injury: A narrative review

摘要


人體的足弓型態在長時間的走路或跑步的情況下,對於整體的足壓分佈以及動態穩定扮演著重要的角色。近年,橫弓被證實與足部勁度息息相關,且橫弓的狀態與前足傷害的發生及預防皆有所關聯,然缺少將過去發表文章的研究成果進行文獻回顧。本篇參照系統性回顧流程進行學術搜尋與篩選,以Web of Science、PubMed、Scopus、SearchWorks及Google Scholar電子資料庫為主,並使用transverse arch與foot及stiffness;transverse arch與forefoot及injury分別進行文章搜索。文獻納入條件包括須以橫弓為主要研究目標,且納入與橫弓有關之元素模型、足部大體以及人體實驗之研究。文獻若是回顧性文章、短篇報導、研討會文章、非學術性文章、醫學用手術方式指引與個案報告則會被排除。經過條件篩選後,總共12篇文章符合條件並進行文獻回顧。由於橫弓的功能支撐人體重量的特性不亞於縱弓,且其型態及動作可以連帶影響縱弓,而橫弓的柔軟度也被視為是良好預防前足疼痛之參考。因此建議未來的研究能夠同時針對橫弓與足部勁度及疲勞或傷害深入探討,有助於介入對於人體足部之影響,如特定足底肌群強化或橫弓支撐應用於足部裝備的設計等,若能對橫弓與足部勁度以及疲勞或傷害有所了解,將有助於介入或設計之應用。

The arch patterns of the human body play an important role in the overall foot pressure distribution and dynamic stability during long-term walking or running. In recent years, it has been confirmed that the transverse arch is closely related to the stiffness of the foot. In addition, the state of the transverse arch is related to the occurrence and prevention of forefoot injuries. However, there is a lack of comprehensive review of the research findings from previously published articles. This study conducted an academic search and screening with reference to the systematic review process, focusing on five databases: Web of Science, PubMed, Scopus, SearchWorks, and Google Scholar. Keywords used for the article search included transverse arch & foot & stiffness and transverse arch & forefoot & injury. The inclusion criteria targeted articles where the transverse arch was the main research focus, and included element models, general foot studies, and human body research. Retrospective articles, short reports, seminar articles, non-academic articles, articles on medical surgical methods and case reports will be excluded. After screening for inclusion and exclusion criteria, a total of 12 articles were included. A literature review of these eligible articles was conducted, and it was concluded that the support of the transverse arch is as important to the human body as the longitudinal arch. The patterns and movements of the transverse arch can jointly affect the longitudinal arch. In addition, the flexibility of the transverse arch is considered a good reference for the prevention of forefoot pain. Future research could extend to the application of transverse arch support. For example, the benefits of specific plantar muscle strengthening or transverse arch support on the transverse arch.
