Surgical Management of Traumatic Auricular Hematoma-A Case Report and Literature Review

摘要

耳廓血腫這個疾病大致可以追溯到古希臘時代就已存在。因為耳廓的突起結構使得它很容易受到鈍傷或穿刺傷，最常見的狀況是肢體接觸的運動、打架或車禍。如果未即時處理，造成畸形，就是菜花狀耳殼。
大部分耳廓血腫都在急性期就被診斷出來，而其他已嚴重變形的大多無開刀矯正意願。文獻上有許多不同的方式來處理這一類的病例，我們報告一例延遲兩周的耳廓血腫病案及技術改良，有幾點強調的是，從後側進入患處對美觀及傷口照顧的好處，包裹式熱塑樹脂石膏之使用及真空吸取式引流等。

Auricular hematoma has a history that dates back to ancient Greece. The prominent position of auricle makes it a candidate of both blunt and penetrating traumatic injury. This occurs most frequently from a rough contact in sports, fighting or motor vehicle accident. If it is untreated, the auricular hematoma may have disastrous results, most notably ”cauliflower” deformity of the pinna.
Majority of the auricular hematomas are diagnosed at the acute stage. The others with obvious deformities rarely intend to receive surgical correction. There have been various methods for management of this problem reported in the literature. We present a case with delayed auricular hematoma for 2 weeks and the modified technique we used for treatment. We emphasize the posterior approach on the purpose of aesthetics and wound care. The application of wrap-around type thermoplastic splint and vacuum drainage are also described in this article.
