Free Thoracodorsal Artery Perforator Flap for Scalp Reconstruction-Two Case Reports
胸背血管穿透枝自由皮瓣在頭皮之重建－兩病例報告
摘要
頭皮缺損之重建對於整型外科醫師而言是一項挑戰，尤其是當惡性腫瘤切除後之廣大範圍缺損。我們報告兩病例，為利用胸背血管穿透枝自由皮瓣在顳枕部頭皮惡性腫瘤手術廣泛切除後之組織缺損重建。其接受血管皆為右側淺顳血管。這兩病例利用胸背血管穿透枝自由皮瓣重建頭皮缺損有幾點優點：頭皮腫瘤切除後之組織缺損較淺，而胸背血管穿透枝皮瓣為一薄及均等厚度之皮瓣，適合頭皮之重建；他並且可提供25×12公分大小之皮瓣；就病人之術位而言，不論腫瘤之切除、取皮瓣及尋找接受血管，病人都是側躺而術中不需再翻身。胸背血管穿透枝皮瓣擁有很多之優點及一些缺點。對於一中至大型頭皮缺損之重建，利用胸背血管穿透枝自由皮瓣將是個不錯之選擇。

Reconstruction of defects in the scalp remains a challenge to plastic surgeons, especially when excising an extensive malignancy. We report 2 cases of scalp malignancies on the temporo-occipital area. Both received wide excision of the tumors. Free thoracodorsal artery perforator flaps were used for reconstruction of defects deep to cranium. The recipient vessels were right superficial temporal vessels. In these cases, there are some advantages for free thoracodorsal artery perforator flap transfer. They are shallow defects after scalp tumors excision. The homogeneous and thin thoracodorsal artery perforator flap is suitable for resurfacing of scalp. The size of thoracodorsal artery perforator flap provides up to 25×12 cm for extensive scalp defect. For the surgical position, to assess the flap as well as the recipient vessels and tumor ablation, they can all be performed in the lateral decubitus position without change. To a medium-to-large scalp defect reconstruction, free thoracodorsal artery perforator flap has benefits over other flaps.
