Striae Distensae after Submuscular Augmentation Mammaplasty-A Case Report

胸肌下隆乳整形後併發萎縮紋－病例報告

摘要

我們在身體上常可以見到萎縮紋的存在，不僅妨礙美觀，而且目前尚未完全清楚它的成因及特定明確的治療方法。隆乳整形後併發萎縮紋的發生在臨床上更是少見。從1981年Snyder首先於文獻中提出後迄今只有十位年輕女性病例報告，僅4例接受治療（長期局部塗抹tretinoin或Vit. E. cream）。我們提出一位19歲長期服用荷爾蒙類抗痘藥物的未婚女性，在接受胸肌下隆乳整形手術後六個月併發萎縮紋，在第8個月的追蹤發現症狀加劇，我們建議立即停用抗痘藥物並施以脈衝光(VL mode)治療（間隔3週共3次療程），2個月之後症狀開始改善，4個月之後症狀緩解病患對治療效果感到滿意。在此我們也建議必須在手術須知及同意書上註記並告知病患（尤其是服用荷爾蒙類藥物之年輕女性）此類罕見併發症的可能性，以避免醫療糾紛的可能。

Cutaneous striae is a common and cosmetically troublesome problem on our skin. However, it is found rarely after augmentation mammaplasty and remains clinical mystery. Up to date, there is not definite treatment and a preventive means. We present a 19-year-old nulliparous female with striae distensae (SD) after submuscular augmentation mammaplasty and try to treat the complication with intense pulse light (IPL) and get satisfactory result. Although SD after breast augmentation is unusual, we have obligation to remind patient of this rare but troublesome complication and review medication history of suspicious case before surgery. Once happened unfortunately, IPL and cessation of steroid therapy can be helpful to resolve such problem.
