Free Thoracodorsal Artery Perforator Flap for Lower Leg and Foot Reconstruction

游離胸背動脈穿通支皮瓣用於遠端小腿及足部重建

於1995年胸背動脈穿通支皮瓣第一次被發表,之後有許多學者發表其對於軟組織重建的許多用途。我們使用游離胸背動脈穿通支皮瓣對於四個病人作遠端小腿及足部的重建。我們於術前使用彩色都卜勒機器對病患的胸背動脈及其皮膚穿通支走向作定位分析,總共作了四個皮瓣,而且所有皮瓣所帶的穿通支皆由胸背動脈下行支所發出。取皮瓣時,我們只取皮膚及以下的皮下組織,並保留擴背肌以及其運動神經。最大的皮瓣面積為18乘以7.5公分。只有一位病患發生植皮瓣區感染,經過一段時間的傷口照顧之後順利恢復,並不影響皮瓣存活。術前彩色都卜勒檢查可幫助定位皮膚穿通支的位置,使手術過程更簡易。游離胸背動脈穿通支皮瓣對於遠端小腿及足部的重建是很好的選擇。

The thoracodorsal artery perforator flap was first described in 1995. Previous authors presented its versatility in soft tissue reconstruction and resurfacing defects. In our study, we used free thoracodorsal artery perforator flap for reconstruction of lower leg and foot defects in 4 patients. We used color Doppler device to evaluate the course of thoracodorsal artery and its skin perforators preoperatively, and 4 flaps were performed. We harvested only skin and subcutaneous tissue, and preserved the latissimus dorsi muscle and its motor nerve. The largest skin paddle size was 18×7.5 cm. All defects were reconstructed successfully. One flap recipient site encountered wound infection. The complication was resolved with a period of wound care, and it didn't compromise the vitality of the flap. Preoperative color Doppler study facilitates identification of skin perforators, thus simplifies the procedure. The thoracodorsal artery perforator flap is a feasible choice for lower leg and foot reconstruction.
