Inverted T Lip Reduction Technique for the Management of Lower Lip Pits in Van der Woude Syndrome

摘要

Van der Woude症候群的患者常合併有唇顎裂，其最明顯的臉部特徵為下唇廔管及下唇突出，而美觀上的考量是一般患者接受手術矯正最主要的原因。根據文獻上的報告，過去有幾種不同的手術方法曾被用來矯正下唇廔管；其中，單純切除法是最被廣泛使用的方法。然而，這些方法的最大問題，在於不能有效而完全的解決患者的問題，以達到美觀的要求，而且常有後遺症的產生。於是在這種情況下，我們設計了一種新的術式-倒T型縮唇術來解決這些問題。事實上，這是一種改良式的下唇整形術。自西元1997年至2002年，在長庚醫院，共有十五位Van der Woude症候群的患者接受倒T型縮唇術來矯正下唇廔管及下唇突出。這其中有兩位年紀較大的患者，小時後曾接受過單純切除術，後來因為結果不滿意而改接受倒T型縮唇術的。在此系列中，所有的患者皆得到了令人滿意的結果並且沒有併發症的產生。這種倒T型縮唇術，根據我們的瞭解，過去不曾有人發表過。因此，本篇文章的目地是要來描述T型縮唇術及探討其臨床應用的情況。從結果上看來，T型縮唇術用來矯正Van der Woude症候群患者的下唇廔管及下唇突出是一簡單而有效的方法，並可達成下唇功能性重建及美觀的要求。

The most prominent facial deformity in patient with Van der Woude syndrome is lower lip pits and lower lip protrusion. Correction of the deformity is the most common indication for surgery. For many years the most common surgical technique was simple excision of the sinuses, which often left significant residual deformities including persistent lip protrusion and irregular white skin roll in the lower lip. To effectively correct these residual deformities, a modified reduction cheiloplasty-the inverted T lip reduction technique was designed to treat the congenital lower lip sinuses since 1997. We present fifteen cases of Van der Woude syndrome and describe this new technique. To our knowledge there have been no reports of the use of this technique in the management of congenital sinuses of lower lip. This reduction technique is simple, safe and effective and it gives a better functional and cosmetic result.
