Total Penis Amputation and Cover the Defect with Anterolateral Thigh Flap-A Case Report

摘要

陰莖外傷或部分截肢是常見的；然而，因自殘導致陰莖完全截肢卻是少見。對陰莖的部分或完全截肢的顯微重建過去曾有些報導，如果斷端無法尋獲則必須作重建手術。已有多種筋膜皮瓣或肌肉皮瓣都曾被用來覆蓋泌尿生殖區的缺損。肌肉皮瓣多用來覆蓋大且深的缺損，其中以股薄肌及腹直肌皮瓣最為常見。然而，對於較淺薄的缺損則適用筋膜皮瓣。我們這個病人是因自殘導致陰莖完全截肢，斷端無法尋獲，於是我們以大腿前外側皮瓣進行重建手術。

Penis trauma or partial amputation is common; however, near-total male genitalia amputation by self-castration is quite rare. Microvascular repair of either total or subtotal penis amputation has been reported(superscript 1,2,3,4). But due to previous psychiatric history and lost of the penile stump, penile reconstruction or replantation was not considered. And the extensive perineogenital defect is reconstructed with the anterolateral thigh flap. Several types of fasciocutaneous and myocutaneous flaps have been introduced to cover perineogenital defects^5. Myocutaneous flaps are commonly recommended for large, deep perineal defects. The gracilis and rectus abdominis myocutaneous flaps have been widely used. However, in superficial defects, use of a less bulky fasciocutaneous flap is indicated.
