Reappraisal of Using Platysma Myocutaneous Flap for Lower Facial Reconstructions

摘要

許多用來重建下半部顏面缺陷的方法曾經被提出。闊頸肌肉皮瓣也是其中一種，優點在於它的顏色相近，厚度夠薄以及皮瓣的順從性。然而它的實用性在自由皮瓣出現後就被低估了。因此本篇研究就是要去回顧我們的經驗中，利用闊頸肌肉皮瓣來重建下半部顏面缺陷的可行性以及可靠性。
回顧從1990到2005年臺大醫院總共有33位病人接受闊頸肌肉皮瓣重建下半部顏面缺陷的手術。所有的病人都對術後的外觀和功能結果感到滿意。
利用闊頸肌肉皮瓣來重建下半部顏面缺陷是個可行而且可靠的方法。不但可以用在腫瘤切除後重建，也可以用在其它方面的重建，包括腐蝕性傷害後疤痕收縮，放射線治療後形成的瘻管，以及口頰壞疽。它提供了良好的外觀和功能上之結果並且合併症尚在可接受範圍內。

Miscellaneous methods have been proposed for reconstruction at the lower facial area. Platysma myocutaneous flap seems to be suitable because of its good color match, thinness and pliability of the skin flap. However, it has been underestimated after free flap presented. The purpose of this study is to review the viability and reliability of the platysma flap for lower facial reconstruction in our experience.
A retrospective review of 33 patients undergoing platysma flap for lower facial reconstruction from 1990 to 2005 in NTUH was conducted. All patients had acceptable aesthetic and functional results after flap reconstruction. Platysma flap is a viable and reliable method for lower facial reconstruction not only after malignancy ablation surgery but also for other lesions, including scar contracture after corrosive injury, post-irradiation fistula, and noma. It provides excellent cosmetic and functional outcomes with acceptable complications.
