Spitz Nevus-An Easily-Neglected Diagnosis
Spitz氏痣－一個容易被忽略的診斷
摘要
Spitz氏痣是一種良性的黑色素細胞病變，與惡性黑色素瘤有相似的組織特徵，常常發生在兒童或青少年。它常見的的特徵，為單一的病兆，呈圓拱形，紅色或棕色的結節。我們報告一位因為右耳良性腫瘤切除手術後，造成隆起疤痕，而前來整形外科門診求助的十歲小女生。我們進行疤痕切片檢查，顯示為Spitz痣。我們為她進行腫瘤切除及全層皮膚移植手術。術後追蹤半年，病患在外觀上獲得令人滿意的結果，同時，無腫瘤再發的現象。在兒童或青少年頭臉部，有不尋常的皮膚隆起及衛星病變，需提高警覺是否為Spitz氏痣的可能。

A Spitz nevus is a benign melanocytic lesion with many histologic similarities to malignant melanoma. A 10-year-old girl visited our outpatient clinic with a non-pigmented and elevated hypertrophic scar about 1.5×1.0 cm over her right ear helix. Approximately 3 months earlier, she had received an auricular tumor excision and two subsequent local steroids by another surgeon. A scar biopsy was done because of the progressive elevation of the lesion, and pathology showed a Spitz nevus. Wide excision of the lesion and then a full-thickness skin graft were done. The cosmetic result was satisfactory without any evidence of recurrence at a six-month follow-up.
