燒炭自殺：一個流行的自殺方式

摘要

近五年來，燒炭自殺成為一種很常見的社會現象。一氧化碳中毒導致的昏迷現象常常讓倖存者遭受到深度的燙傷或是神經功能的缺損。為了讓深度的傷口得以快速恢復往往需要移植健康的組織來覆蓋。我們報告一個28歲女性病人，燒炭獲救之後,治療膝蓋四度燒傷的經過。在同一次清瘡手術後利用股外側皮瓣覆蓋裸露的關節囊及四頭肌肌腱，利用膝蓋附近的血管作為皮瓣循環的來源。手術不用改變姿勢即可以完成。文中並說明燒炭自殺近年的流病歷程。

In the recent 5 years, there is an emerging local trend of burning charcoal as a form of suicide attempt. Patients who committed suicide usually suffered from carbon monoxide inhalation lung injury and severe deep contact thermal burns if direct fall down on the burning charcoal. Patients are usually admitted to the burn unit and received burn wound management. Early wound closure and better functional results are the goals. We reported a female patient who committing suicide by burning charcoal. Her anterior and lateral knee joint suffered from the 5% body surface third degree burn injury. Very early tangential excision was performed and free anterolateral thigh cutaneous flap (28x21 cm) was transferred immediately after wound debridement to cover the joint capsule and quadriceps tendon. The patient recovered both emotionally and physically and was discharged 18 days after the admission.
