中風患者之上肢動作功能對其日常生活功能與生活品質的影響

The Influence of Upper Limb Motor Function on the Activity of Life and Quality of Life in Stroke Patients

摘要

目的：本研究嘗試探討中風患者之上肢動作功能對其日常生活功能與生活品質之影響，也欲了解動作損傷程度不同其上述表現是否有差異存在。方法：本研究共收取57位中風患者（平均年齡55.55±13.11歲，發病時程1.57±1.87年），使用傅格－梅爾評估量表（Fugl-Meyer assessment, FMA）上肢功能分測驗來了解個案上肢功能，功能獨立量表（functional independence measure, FIM）與健康相關之生活品質問卷－中風影響量表（stoke impact scale, SIS）來了解患者的日常生活獨立性與生活品質。結果：結果顯示上肢動作功能（FMA）與FIM總分，以及自我照顧、移動能力兩分項具有顯著正相關（p=0.012, p=0.007, p=0.04）；除記憶力與溝通能力兩分項外，FMA也與SIS其餘6分項具有顯著正相關（p＜0.05）。隨上肢損傷程度加重，中風患者的日常生活功能與生活品質亦會受到負向影響；上肢功能輕度損傷之中風患者的生活功能與自評生活品質明顯優於中度與重度損傷者。但因生活品質所涵蓋的層面極廣，為有效促進患者的生活品質表現，應更全面性的考量，也需要未來研究進一步探討。

Objective: This study attempted to investigate the influence of upper limb motor function on activities of daily living(ADLs) and quality of life(QoL) in stroke patients. We also wanted to reveal whether a disparity exists in ADLs and QoL for patients with different upper limbs impairment. Methods: A total of 57 stroke patients (mean age=55.55±13.11 years old, onset time= 1.57±1.87 years) were analyzed in the study. Methods used to evaluate outcomes included Fugl-Meyer assessment (FMA) for assessing the motor function of upper limbs; functional independence measure (FIM) for assessing the independent of ADLs, and stoke impact scale (SIS) for evaluating the QoL. Results: Correlations were found between FMA and FIM (p=.012), and two subtests of FIM (self-care and mobility, p=.007 and p=.004). Except for memory and communication, there was also a positive correlation between FMA and the other 6 domains of SIS (p＜0.05). Conclusion: As the degree of upper limb injury get worse, the ADLs and QoL of stroke patients would be negatively affected; patients with mild upper limbs impairment would have better ADL function and QoL than those of moderate and severe injuries. However, because the QoL covers a wide range of aspects, in order to improve the QoL performance of patients effectively, we should consider more comprehensively in the rehabilitation programs. Further studies to clarify this actual correlation are also required.
