中風患者單側忽略的定義特徵之臨床效度測定

摘要

本研究爲一探索性研究，以42位中風病人爲對象，利用自擬的「單側忽略定義特徵評量表」探討病人單側忽略的行爲，利用Cronbach's alpha 係數、t-test、ANOVA、及因素分析等統計方式，結果顯示12項定義特微的Cronbach's alpha係數爲0.85。病人的基本資料以患側上肢的本體感覺、輕觸覺功能、運動功能、痛覺、中風位置、視覺功能、聽覺功能等七項對單側忽略的定義特徵具有顯著的影響，而前三項的解釋變異量達44.06%，可作爲單側忽略的預測因子。經因素分析後，可用「視覺空間忽略性表徵」與「患側身體忽略佳表徵」二個次概念涵蓋，其總解釋量爲52.0%。再依中數值、矯正單項／總項相關係數、因素固有值、因素係數等四個條件選出5項必要性定義特徵，可做爲護理人員正確迅速評估之依據。

Unilateral Neglect (UN) is a commom health problem in brain deficit's patients. The purposes of this study are (1) to establish the defining characteristics (DC) of UN, (2) to establish the reliability and construct validity for the DC of UN, (3) to identify the essential DC of UN, (4) to explore the affected factors of the DC of UN.
The investigator directly assessed 42 stroke patients in terms of 12 DC in a 5-point rating scale. The data was collected based on observation, interview, physical examination, and chart review. Results of this study showed that 12 DC had a desirable Cronbach's alpha coefficient 0.85 and a significant average inter-item correlation 0.32. Corrected item/total correlations of all items were over 0.30. Based on factor loading ≧0.40 two factors were extracted from these DC in terms of orthogonal factor analysis that sharing the total explained variance 52.0%. The two factors were labelled ”visual spatial neglect” and ”affected body neglect”. Five DC are selected by four criteria which revealed eigenvalue ≧1.0, significant factor loading ≧0.40, corrected item/total correlation ≧0.30, and the overall median score of the total scale.
