安寧護理模式於改善癌症病患家屬憂傷反應之成效探討

安寧護理秉持人性化之原則，協助臨終病患與家屬由瞭解與接受死亡，進而珍惜有限人生，於尊嚴與肯定中結束人生。本研究之目的在探討安寧護理模式於改善癌症病患家屬憂傷反應之成效。本研究採類實驗法中之非同時問設計法執行，針對某綜合醫院1992年10月至1993年7月間，以該院首次罹患癌症之家屬列爲研究對象，採配對方式取樣50名後分爲兩組：實驗組中樣本除接受傳統醫療與護理服務外，另接受六位經訓練之護理師提供每週五次，每次一小時之護理治療活動；對照組中樣本僅接受傳統之醫療處置與護理服務，治療成效將以治療前、後，家屬所填寫之憂傷反應調查表之得分爲指標。研究資料將採ANOVA與描述統計法處理之。研究結果顯示：安寧護理於改善家屬於憂傷反應中之焦慮、退縮、無力感與絕望行爲有顯著成效，此模式可作爲提昇專業自主與改善臨終護理服務品質之參考。

Hospice care which based on humanistic principle is a new trend of care to terminal ill. The model is to help hopeless patients and their families to accept death, as the ending: of life process. The purpose of the study. was to examine the effects of a nursing model for hospice care on: improving. grief responses of families of hospitalized cancer patients. A two-group, non-synchronized method of quasi-experimental design was conducted. A sample of 50 newly diagnosed cancer patients and families were selected randomly through October 1992 to July 1993 and assigned by meshing to one of two groups. Treatment group subjects in the group received 10 sessions of hospice care program which was conducted 1 hour per session and 5 times a week by one of six trained nursing therapists. Control group: With out the above program. Subjects in both group received routined services and nursing care. The tool is self-developed grief response inventory. The most significant contribution of the study is to improve grief reactions to the families of cancer patients and to enhance professional autonomy.
