摘要

本文描述一位母親在護理人員的指導下，成功的運用袋鼠式護理照顧雙胞胎早產兒的心路歷程。筆者以實際參與照顧的方式包括照護期間的訪談，家中訪視及電話訪談收集資料，將母親與雙胞胎早產兒互動之語言和非語言行爲記錄成爲八篇行爲過程實錄。採用內容分析法將資料整理歸納出五項主題，包括「獻出自己」、「認識寶寶」、「發展依附」、「確爲人母」、「與配偶共享喜悅」。在執行袋鼠式護理期間，母親可以觀察孩子的生理及行爲反應，不但可以掌控嬰兒情況，而且在照料上可以增加信心、減輕焦慮、加強親子間的依附、以及有利於早產兒睡眠安穩和體重增加。本個案報告驗證護理人員在母親面對雙胞胎早產兒時，營造一個讓母親參與照護的環境以及協助母親執行袋鼠式護理而獲得信心和正向的產後經驗，期望本文可以提供護理專業人員在早產兒家庭照護上的參考。

This article described maternal experience of kangaroo care with a premature twin under the nursing instruction. Data collection was conducted by participated observation and telephone and in-home interviews. The verbal and non-verbal interacting behaviors between mother and premature twins were transferred to eight process recordings. Using content analysis, five themes were inducted, including giving oneself, recognizing the baby, developing bonding, being a mother, and sharing happiness. During the period of practicing kangaroo care, the mother could observe the physiological and behavioral responses from the babies. She could manage all situations related to the baby and also improve self-confidence, reduce anxiety, and increase parent-infant attachment that helped the babies sleep well and increase body weight. This case report confirms nursing function to build up a caring environment for a mother to conduct the kangaroo care and improve the mother's self-confidence. Hopefully, this article could be as a reference for nursing professionals offering premature baby care.
