摘要

隨著社會型態及家庭結構改變，老人遷居到長期照護機構的情況已越來越普遍。本文描述一位初次遷居護理之家老人之護理經驗，自2014年3月7日至5月6日護理期間，以觀察、會談及身體評估等方式收集資料，並運用Gordon十一項功能性健康型態評估，確立個案之護理問題有：遷移壓力症候群、營養不均衡-少於身體需要及便祕。在照護過程中，筆者依個案個別性提供活動安排、飲食設計及衛教指導，協助其適應遷居生活、改善營養不均衡並養成規律排便習慣。期望此護理經驗能做為護理人員照顧初次遷居護理之家老人參考。

With changing social and familial structures, the situation of elderly relocating to a long-term care facility has become more and more common. The purpose of this article was to explore the nursing experience for an elderly adult undergoing first-time relocation to a nursing home. From March 7 to May 6, 2014, data were collected through observation, interview, and physical examination according to Gordon' functional health patterns. The nursing problems identified included relocation stress syndrome, imbalanced nutrition: less than body requirements, and constipation. By providing individualized living arrangement, dietary design, and health education, the author helped the adult adapt to the lives of relocation, improve the nutritional imbalance, and cultivate regular bowel habits. This nursing experience can serve as a reference for caring elderly adults undergoing first-time relocation to nursing homes.
