收治一位疑似新型冠狀病毒肺炎之急性精神科病患防疫經驗

Lessons Learned from an Acutely Psychotic Patient with Suspected COVID-19 Infection

摘要


新型冠狀病毒肺炎(COVID-19)為一項新興傳染疾病,擁有高度傳染力,亦於世界各地擴散中,迄今已高達兩百萬人次因此疾患所致死亡,為防範醫療機構中病患院內交互感染風險,擬定一系列防疫因應策略為其重要。本文個案為疑似感染新型冠狀病毒肺炎之精神科急性患者,於北部某醫學中心啟動急性精神科病房防疫應變小組,建構標準化作業流程,闡述病患由急診室辦理精神科病房住院程序,以及入住病房之防疫流程,包含:醫護專業人員防護系列方針、單獨病室處置照護方式及解除隔離後之病人個別化措施等。照護期間為2021年01月23日至2021月02月08日,藉動態時間軸方式回顧一系列急性精神科病房防疫考量,希冀此經驗能提供給精神衛生從業人員,當處遇或照顧合併疑似傳染病風險個案之防護策略方針參考及應用。

Coronavirus disease 2019 (COVID-19) is a novel disease that is highly infectious and globally widespread. To date, it has caused approximately 2 million deaths. It is thus critical to develop COVID-19 preventing strategies to prevent infections at healthcare institutions. This case study is based on an acutely psychotic patient at a medical center in northern Taiwan with suspected COVID-19 infection. A COVID-19 preventing team was activated at the acute psychiatric ward, which developed standard operating procedures for admitting patients into psychiatric ward, as well as COVID-19 prevention procedures in the ward. These procedures included protection guidance for healthcare professionals, method of care in the single-bedded isolation room, and individualized post-isolation approaches for the patient, among others. The care period of this patient was between January 23, 2021 and February 8, 2021. COVID-19 prevention procedures in the acute psychiatric ward were reviewed using dynamic time warping. This experience can serve as a useful reference and guideline for mental health professionals when treating or caring for patients with suspected COVID-19 disease.
