照護一位戒斷症候群病嬰之護理經驗
Nursing Experience of caring a newborn with Neonatal Abstinence Syndrome
摘要
本文描述一位新生兒因母親懷孕期間吸食海洛因產生藥物戒斷症候群之照護經驗。護理期間運用系統性身體評估、新生兒戒斷評分量表（NASS）評估、直接照護等方式收集相關資料，確立病嬰有「混亂性嬰兒行為」、「腹瀉」、「家庭因應功能失調」等健康問題。護理期間除了提供新生兒戒斷症狀的醫療處置外，並給予支持性照護，以緩解混亂性行為與腹瀉，因案母產後需入獄服刑無心撫養病嬰，遺棄新生兒自行離院，透過跨團隊尋找資源，讓個案能妥善得到後續照顧。建議照護新生兒戒斷症候群需要醫護間完整的評估及敏銳的觀察能力，適時介入資源，期望藉由本個案提供護理同仁更完善的照護，進而提升照護品質。

This article described the nursing experience of caring a newborn with Neonatal Abstinence Syndrome (NAS). Systematic physical assessment, Neonatal Withdrawal Score Scale (NASS), and direct care were used to collect information. The health problems identified included chaotic baby behavior, diarrhea, and family instability. The NAS hospitalization and supportive care were provided to lessen the discomfort of chaotic behavior and diarrhea. The neonate was abandoned soon after birth as the mother fled from the hospital to avoid going to prison. Combined resources from multiple divisions were needed. To provide quality nursing care for such a complicated patient, keen assessment and timely intervention are required.
