本文描述一位複雜性腹裂新生兒護理經驗,個案出生後即進行手術,住院過程中除面臨生理的健康問題外,案母為外籍母親,有著語言的隔閡及認知的差異,案父母也因對疾病進展與治療不瞭解,對照護個案之能力感到擔憂,導致心理衝擊及壓力,引發筆者欲深入了解。照護期間自2019年3月13日至2019年3月31日,藉由直接照護個案、臨床觀察、與案父母會談、整體性健康評估等方式收集個案資料,確立健康問題有:急性疼痛、營養少於身體需要、以及照顧者角色緊張。護理期間依個案生理需求擬定個別性照護措施,逐步改善個案的健康問題;案父母擔心個案病情及後續照護問題,協助父母了解及鼓勵參與照護活動,成功的減輕照顧者角色緊張的問題。建議於臨床除了解決個案生理需求外,增加與父母互動時間及傾聽技巧,進而降低父母焦慮及擔憂,故期望藉由此護理經驗,提供護理人員照護複雜性腹裂新生兒之參考,提供適切的個別性照護,以提高護理品質。
This article describes the post-operative nursing experience of a neonate with complicated gastroschisis. The patient received surgery immediately after birth. During the hospitalization, the patient has physical health problems. The mother who was a foreign national has language barriers and difficulty cognition to the disease. The parents are worried about their ability to care for the patient due to unfamiliarity with the disease progression and treatment. During the caring period from March 13, 2019, to March 31, 2019, the patient's data were collected through clinical observations, interviews, systematic assessments, and family assessments. Health problems were identified as acute pain, malnutrition, and caregiver role strain. During the nursing period, the care intervention was planned individually according to the needs of the patient. The physical health problem of the patient was gradually improved. As for caregiver role strain, the nurse assisted the parents to understand and participate in care activities and caring skills learning. Parents' care stress is decreased. It is recommended that nurses are not only solved patient's physical health problem but also enhance the time and skills of interaction with family members. The case report may serve as a reference for future nursing staff when caring for a newborn with complicated gastroschisis. We hope the nurses can apply appropriate individual nursing care to enhance the quality of care.
