照護一位遇家庭暴力未成年初產婦之經驗

本文描述照護一位遭受家庭暴力未成年初產婦之護理經驗，護理期間為2019年3月12日至2019年3月15日，筆者以訪談、觀察、查閱病歷等方式收集資料，發現個案主要護理問題有：恐懼、低效性母乳哺餵、家庭運作功能失常，並運用Watson關懷理論進行護理活動，同理、傾聽、陪伴達到持續性心理上的支持與鼓勵，提供安全隱密的環境及降低恐懼的因應措施，增進其母乳哺餵及新生兒照護技巧，並由社工師、關懷師介入居中協調、溝通，提供相關社會資源，讓個案能即早進入母職的角色，並培養照護新生兒的責任與能力。由於住院天數短，出院後僅能以電話追蹤關懷個案情形，建議透過早期介入及持續性的長期關懷，協助脫離家庭暴力的陰影及調適為人母的角色。藉此照護經驗與臨床同仁分享，作為日後照護此類個案之參考。

This article describes the care experience of a woman who has suffered from intimate violence at the beginning of the year. Duration of the nursing care was between March 12 to March 15, 2019. The author collects data by interview, observation, and medical records, and finds the main nursing problems of the case. The nursing problems identified included: fear, inefficient breastfeeding, and interrupted family processes and using Watson care theory to carry out nursing activities, empathy, listening, companionship to achieve sustained psychological support and encouragement, providing a safe and intimate environment , reducing fear In response , measures to improve breastfeeding and neonatal care skills. Social workers and chaplain are involved in coordination, communication, and provide relevant social resources, so that cases can develop responsibility and ability to care for newborns to enter the role of mother and child care soon. Owing to the short length of hospital stay, we using phone follow-up case situation after being discharged from the hospital. It is recommended early intervention and sustained long-term care to help break away from the shadow of intimate violence and adapt to the role of motherhood .Here we share our experience for taking care of this kind of patients in the future.
