運用跨文化理論照護一位創傷性骨盆骨折外籍個案之護理經驗
Nursing Experience of a Foreign Patient with Traumatic Pelvic Fracture using Transcultural Nursing Models
摘要
本文為探討一位獨自來台貿易經商，工作時不慎由兩樓高的貨櫃上跌落，導致創傷性骨盆骨折的外籍個案之護理經驗。筆者於2019年2月21日至3月15日，藉由Leininger的日出文化照護模式進行身體評估後，分析個案在文化適應上的衝突包括：飲食習慣、宗教融入日常的生活模式、對身體自主的強烈維護等，確立有潛在危險性危及個人尊嚴、身體活動功能障礙、焦慮等三項護理問題。護理過程中，發現個案在文化價值觀及宗教方面與台灣有許多差異，故照護過程必需以尊重且不與文化衝突為原則來給予護理措施，才能得到適切的照護，在深入了解文化背景與宗教特性後，藉由文化照護的調整及再造，與彼此協議及達成共識，如說服能接受醫院餐點並採均衡飲食、用右手使用湯匙及叉子進食、並嘗試用手機將麥加天房圖片顯示作禮拜等措施，逐漸緩和焦慮情緒與卸下心防，並運用跨領域聯合討論會，得知個案有想早期復健、盡快出院等目標，由物理治療師依個別性與個案共同討論及訂定早期復健運動等訓練計畫，終使能獨自使用助行器下床行走，並於2019年3月15日順利出院。藉由此次跨文化照護理論的精神及重要性之照護經驗，能提供未來類似個案之參考。

This article discusses the nursing experience of a foreign businessman who accidentally fell from two-story-high trailer, resulting in a pelvic fracture. Using the Leininger's Sunrise model, we evaluated the patient from February 21 to March 15, 2019. We analyzed the difficulties of cultural adaptation including eating habits, religion, patient autonomy, dignity, anxiety. There were many cultural and religious differences that required adaptation during nursing care and required various adjustments and understanding of the patient's own culture and religion. Examples include persuading the patient to tolerated the hospital meal and acquire a balanced diet, using the right hand to hold utensils, using pictures of Mecca on the smartphone to help the patient pray when unable to leave the ward. These adjustments helped calm the patient, improve trust and motivated the patient toward early rehabilitation and successful discharge on March 15, 2019. This nursing experience demonstrated the importance of Transcultural Nursing Models congruent nursing care.
