Primary Cutaneous Lichen Amyloidosus Following Lines of Blaschko

摘要

一位57歲男性，1年前在雙腿側面及前脛出現多法性且具癢感之角化丘疹。在雙側前脛之丘疹呈線性分佈，並且沿著Blaschko氏線分佈。組織病理學下可見於真皮上層有圓形嗜伊紅性物質存在，以剛果紅染色呈陽性反應。在電子顯微鏡下可見6至10mm寬之線性。無分支管狀纖維舒鬆排列成網狀結構。證實類澱粉之存在。我們使用terminal deoxynucleotidyl transferase-mediated deoxyuridine triphosphate nick end labeling〈TUNEL〉方法來偵測檢體中凋零之表皮細胞，且其結果為陽性。依據臨床，病理及電子顯微鏡下之發現，我們確定此為一以罕見線性分佈之類澱粉性苔蘚之病例。

A 57-year-old male presented with a one-year history of multiple pruritic discrete hyperkeratotic papules that first appeared on his bilateral shins and thighs symmetrically. Lesions were arranged in a distinctive linear pattern on his bilateral shins, which appeared to follow Blaschko’s lines. Histologically, round eosinophilic globules were found in the upper dermis, which stained positively with Congo red. Electron microscopy revealed 6 to 10 nm wide, linear, nonbranching tubular fibrils loosely arranged in a meshwork, proving the presence of amyloid. We used a terminal deoxynucleotidyl transferase-mediated deoxyuridine triphosphate nick end labeling (TUNEL) method to detect the apoptotic keratinocytes within specimen, and the result was positive. Based on the clinical, pathological and electron microscopic findings, we confirmed that this is a case of primary cutaneous lichen amyloidosus with a rare linear distribution.
