Majocchi's Granuloma Caused by Trichophyton Rubrum in a Renal Transplant Recipient -A Case Report and Review of Literature-

摘要

一位31歲男性病患，一年多前由於慢性腎衰竭接受腎臟移植。一個月前在腳趾背部及左小腿陸續出現深紅色到棕色表皮下結節。病理切片檢查顯示為發炎性肉芽腫反應，Periodic acid-Schiff染色下顯示許多黴菌菌絲在真皮中，同時三週後皮膚組織及膿的黴菌培養出紅色毛癬菌。在我們使用terbinafine 250 mg三週後，病灶改善許多。因此對於接受免疫抑制濟的慢性黴菌感染的患者，若出現皮下結節，必須要考慮到Majocchi’s granuloma的可能性。

A 31-year-old male who received renal transplantation due to chronic renal failure more than one
year ago, presented with several dark reddish-brown subcutaneous nodules on the dorsum of his toes and
left lower leg for 1 month. A skin biopsy specimen revealed an inflammatory granulomatous reaction,
and several fungal hyphae were revealed by periodic acid-Schiff stain. Trichophyton rubrum was isolated
3 weeks after fungal culture of the skin tissue and pus. He received terbinafine 250mg treatment for 3
weeks and the lesions improved much. When subcutaneous nodules were found in a patient who received
immunosuppressive drugs with known chronic fungal infection before, we must keep in mind the
possibility of Majocchi’s granuloma.
