Frequency of Occurrence and Susceptibility of Pathogens Associated with Furuncle and Carbuncle-A Study in a Regional Hospital of Central Taiwan

癤和癰的致病菌發生率及其抗藥敏感性－中臺灣－區域醫院之研究－

摘要

秀傳紀念醫院皮膚科門診在2004年內，從癤和癰的病人中分離出總共85株細菌。致病菌的順序依序為金黃色葡萄球菌(52.9%)，陰性凝固酶葡萄球菌(16.5%)，鏈球菌(4.7%)，變形菌(4.7%)，克雷白桿菌(3.5%)，Citrobacter sp.(3.5%)，大腸桿菌(3.5%)，不動桿菌(3.5%)，Peptostreptococcus(1.2%)，腸桿菌屬(1.2%)，綠膿桿菌(1.2%)，巴斯德拉菌屬(1.2%)，Porphyromas(l.2%)，和眞桿菌(l.2%)。我們用盤擴散方法分析了所有臨床菌株對常用抗生素的敏感性試驗。在金黃色葡萄球菌中，對盤尼西林、紅黴素、氯林絲菌素的抗藥性分別為91.1%、84.4%和84.4%。值得注意的是，31.3%的金黃色葡萄球菌對oxacillin有抗藥性。對萬古黴素、teicoplanin和fusidic acid的抗藥菌株並沒有被發現。對於革蘭氏陰性菌種，ceftriaxone、艾米康絲菌素和imipenem均展現了優良的抗菌能力（大於95%敏感性）。這篇報告的結果分離出台灣區域醫院皮膚科門診癤和癰最常見的致病菌，其抗生素敏感性試驗結果並可作為本地區癤和癰的經驗療法。

A total of 85 bacterial isolates were recovered from furuncle and carbuncle from the dermatology clinic at Show-Chwan Memorial Hospital in 2004. The rank order of pathogens was: Staphylococcus aureus (52.9%), coagulase-negative staphylococcus (16.5%), Streptococcus sp. (4.7%), Proteus (4.7%), Klebsiella (3.5%), Citrobacter sp. (3.5%), E. coli (3.5%), Acinetobacter sp. (3.5%), Peptostreptococcus (1.2%), Enterobacter sp. (1.2%), Pseudomonas aeruginosa (1.2%), Pasteurella (1.2%), Porphyromas (1.2%), and Eubacterium sp. (1.2%). We analyzed the antimicrobial susceptibilities of all clinical isolates to the commonly used antimicrobial agents by the disk diffusion method. Of the Staphylococcus aureus isolates, resistance to penicillin, erythromycin, and clindamycin were 91.1%, 84.4%, and 84.4%, respectively. Of note, 31.3% of Staphylococcus aureus were oxacillin resistant. No resistance to vancomycin, teicoplanin or fusidic acid was found. For gram-negative organisms (GNO), ceftriaxone, amikacin and imipenem showed an excellent spectrum of activity (＞95% susceptible). The results of this study have identified the most common pathogens of furuncle and carbuncle in a regional hospital in Taiwan, and can be used for empiric treatment for furuncle and carbuncle in this region.
