The severely atrophic edentulous ridge has been a challenge not only for the placement of the implant but also for the rehabilitation of the prostheses. Efforts have been made to increase the vertical height of bone. The transmandibular implant system is indicated for reconstruction of the severely atrophic mandibular edentulous ridge without bone augmentation or vestibuloplasty. The purpose of the paper is to introduce transmandibuar implant, including materials design, and clinical procedure. Both advantages and disadvantages are also mentioned.
