A case of labial fenestration in a mandibular right central incisor is described. The patient was a 35-year-old female seeking regular dental examination. Following conventional endodontic therapy, a labial fenestration appeared at the tooth apex due to overfilling of root canal. A pedicle flap was utilized to successfully repair the mucosal/bony defect after thorough periapical curettage and apicoectomy of the root apex. Nine years postoperatively, the mucosa has remained covered, and there was radiographic evidence of bone infilling in the periapical region. This cause of labial fenestration is very rare. The case report discusses its possible pathogenesis and treatment procedures to provide clinicians an alternative method of treatment.
