摘要


牙周治療的最終目地是:對新骨、新牙骨質及新的牙周韌帶有再生的作用。本篇研究的目地係評估『牙周的骨缺損中』,植入同種異體冷凍乾燥骨的效果。實驗共選取十個病人之二十個部位、其牙周囊袋的深度在4-10 mm,全部牙周的骨缺損中約接受移植同種異體冷凍乾燥骨的再生手術。臨床評估包含術前、術後六個月、及十五個月利量牙周囊袋的探測深度、牙齦萎縮量及臨床附連的程度,以分析牙周狀態改善之情形。結果發現牙周探測囊袋在術後六個月平均減少3.5 mm及在十五個月時則減少3.16 mm;在臨床附連程度方面:在術後六個月增加3.83 mm、在術後十五個月增加3.49 mm。這個統計的結果証實了在牙周的骨缺損中,使用同種異體冷凍乾燥骨移植,的確可以達到改善臨床牙周缺損的效果,然本實驗未能包含隨機對照組實屬可惜,而改善牙周臨床效果下之確切組織結構,也值得做進一步之探討。

The ultimate goal of periodontal therapy is the reconstruction of periodontium by regeneration of new bone, cementum, and periodontal ligament. The purpose of this study was to evaluate the effect of demineralized freeze-dried bone allograft (DFDBA) for periodontal angular bony defect. Ten patients, whose probing depth ranged from 4-10 mm at 20 defect sites, received DFDBA regenerative surgical therapy. Probing depth, clinical attachment level, gingival recession were measured at baseline, 6-month, and the 15-month after operations. The results showed that the reduction of probing depth was 3.50 mm at 6-month and 3.16 mm at 15-month.The mean gain in clinical attachment level was 3.83 mm at 6-month and 3.49 mm at 15-month. These results demonstrated that DFDBA could provide a significantly effect for the periodontal therapy, and these improvement were maintained over the course of the study. However, the further study about randomized control group was needed in the future.
