能否利用共振頻率分析判斷植體何時可以開始受力?病例報告

Can Resonance Frequency Analysis be used to Determine When to Load on an Implant? A Case Report

摘要


骨整合(Ossointegration)是決定植牙成功與否的基本要件。由於植體表面處理方式的改變,以及臨床技術的進步,有效地縮短了骨整合的時間。臨床上有許多方式用來檢查植體植入後的穩定性,可作爲臨床醫師決定植體開始接受荷重時機的參考,而共振頻率分析(resonance frequency analysis, RFA)被認爲是一種較可靠的利量方式。在此提出一個病例,患者於上顎左側第二大臼齒區植入一顆ITI(上標 ®)10mm長的植體,植入植體後以及術後十週,均利用共振頻率分析利量植體穩定度,皆利得高且穩定的數值(ISQ介於76至80),期間病患亦無任何不適症狀,卻在支台齒鎖緊之際,產生植體旋轉的情況,當下製作暫時性樹脂套帽保護支台齒,且避免任何的咬合接觸,靜待四個月後,再度鎖緊支台齒至35Ncm,並重新置入固定義齒,追蹤十個月至今,使用狀況良好,放射學檢查亦無明顯骨吸收的現象。希望藉此病例,來探討植體旋轉可能發生的原因和處理方式,以及討論RFA在臨床應用上的注意事項。

The success of dental implant is highly dependent on osseointegration, which contributes to implant stability. The clinicians determine the timing of loading to the implant according to the implant stability. Despite there are various methods of measuring the stability of implants, resonance frequency analysis (RFA) is thought to be one of the most objective, reliable, and less invasive methods so far. However, the correlation between the ISQ values and the degree of implant stability is still controversial. In this case report, a 48 year-old man received a 10mmITT(superscript ®) Stramman dental implant on maxillary left second molar area with a series of high and stable RFA values thereafter. Implant rotation occurred during the abutment tightening. Temporary resinous coping free from occlusion was fabricated to protect the abutment. After close follow-up recalls for 4 months, the abutment was torque to 35Ncm again and a crown was delivered without any symptoms and signs. Radiographic exam did not reveal obvious bone resorption or defect. The crown has been in function for 10 month well until now. The possible causes and the prevalence of implant rotation along with the clinical implication and limitation for resonance frequency analysis were discussed.
